Submit a current certificate of insurance (COI) and your business registration — your formation certificate, plus your entity must be in good standing with the state. The Named Insured on the COI must be the exact legal name on your Certificate of Formation / Texas registration (not only a DBA). If you operate as an individual (no LLC or corporation), you verify with a government-issued photo ID in your legal name instead of a business registration. We review these before you can accept jobs. If you're between policies, get covered first — we can't waive this one.
